Immunization Reminder

Reminder for parents/guardians:
 
During these uncertain times, we are aware that parents and guardians are doing all they can to keep their children healthy and safe. As a reminder, the State of Iowa still requires proof that immunizations are up to date, or a state allowed exemption be on file, for students to be able to attend school. Local health care provider's offices are open and are ready to provide immunizations. Please contact your healthcare provider as soon as possible to make your child an appointment if they are not up to date, or if you are unsure. Your health care provider can provide you information about how to keep your child safe when receiving their immunizations. Grade level requirements:

 

Preschool/AK:

·        Dtap: 4 doses

·        Polio: 3 doses

·        HIB (hemophilus influenza): 1 or 3 depending on age first received 

·        Pneumococcal: 1 or 4 depending on age first received

·        MMR: 1 dose

·        Varicella: 1 dose or history of disease

Kindergarten through 12th grade (number of doses can vary depending on circumstances):

·        Dtap: 5 doses

·        Polio: 4 doses

·        MMR: 2 doses

·        Hepatitis B: 3 doses

·        Varicella: 2 doses or history of disease

7th and 12th grade additional requirements:

7th grade: Tdap: 1 dose

                   Menigococcal: 1 dose

12th grade: Tdap: 1 dose  (if did not already receive)

                     Menigococcal: 2 doses; or 1 dose if the student recieived the first dose after the age of 16.
