Important Vaccine REMINDER for incoming 7th and 12th Graders

Before 7th and 12th-grade students can begin school in the fall, they must meet mandatory state vaccination requirements. Students without proof of vaccination, or a state medical/religious exemption, will not be allowed to attend classes. Details are below.

The Iowa Department of Public Health requires:

Students entering 7th grade:

  • 1 dose of meningococcal vaccine  (if born after September 15, 2004) received on or after age 10
  • 1 dose of Tdap (tetanus, diphtheria, acellular pertussis) vaccine, (if born after September 15, 2000)

Students entering 12th grade:

  • 2 doses of meningococcal vaccine (if born after September 15, 1999), or one dose if received when the student was 16 years of age or older
  • 1 dose of Tdap (tetanus, diphtheria, acellular pertussis) vaccine, (if born after September 15, 2000)

Please contact your primary healthcare provider regarding these required vaccines now, and submit the completed forms to the school nurse or health secretary at your child’s school as soon as possible – you can return these forms before the end of this school year.
