BASIC GROUP TERM LIFE INSURANCE

The District provides a basic level of group term life insurance coverage at no cost to benefit eligible employees.  This coverage is a term life benefit, meaning that it does not earn cash value and the coverage is terminated when employment ends.  There are options for the employee to continue the term life coverage directly with Reliance Standard Insurance when employment ends.

ENROLLMENT

This benefit is provided by the District at no cost to eligible employees and enrollment is automatic.  Eligible employees must complete a Designation of Beneficiary form and provide it to the Benefits Department.  Beneficiary designations can be changed at any time.

SUPPLEMENTAL AND DEPENDENT LIFE INSURANCE

TERMINATION OF COVERAGE – BASIC, SUPPLEMENTAL & DEPENDENT LIFE


PORTABILITY

You may apply to continue your Supplemental and Dependent Life Insurance, subject to the plan provisions.  You must apply for portability within 31 days of the date your insurance terminates.  You may elect to decrease the amount of coverage you wish to port, but you will not be eligible to increase the amount of ported coverage.  The minimum amount of insurance that you can apply to port is $5,000.

CONVERSION

You may convert this insurance by applying for and paying the first premium for an individual policy within 31 days after any part of your Life Insurance under the Group Policy stops subject to the plan provisions.  Proof of good health is not required.
